1.            SERVICE HOURS
 

Monday to Friday :          9:00 a.m. – 12:30 p.m.

                                           2:00 p.m. – 5:00 p.m.
 

Saturday, Sunday & Public Holiday :     Closed


 

2.            ADMISSION

 

Appointment should be made prior to individual or organizational visit.

 

 

3.            SERVICES

 

To promote the importance of healthy lifestyle, particularly in respect of tuberculosis, chest and heart diseases, the following services are available in the Centre:

 

-              Health Talk and Workshop

 

-              Health Check (blood pressure, body fat ratio and body mass index)

 

-              Health Information

 

-              Health Booklet and Leaflet

 

 

4.            MEMBERSHIP APPLICATION

 

Membership category and fee

 

Life member : HK$1000                    Senior  member (aged 65 or above) : HK$90

Ordinary member : HK$180            Student member : HK$90

 

Membership regulations

 

-               Applicant of student member should be a full-time student.

 

-            The expiry date of all kinds of memberships is on 31st December of the year of application. (Except for Life member)

 

-               Membership fee is not refundable. (Except for those applications rejected by the Centre)

 

-               Members would enjoy special rates for the Centre activities.

 

-               Members would receive the newsletter of the Association and information of the Centre activities.

 

Methods of application

 

In person: Please submit the completed application form with membership fee to the Centre during the service hours, i.e. 9:00 a.m. - 12:30 p.m. and 2:00 p.m. - 5:00 p.m. For the application of senior and student member, please show your ID card/ Senior Citizen card or student card to the staff for checking.

 

By mail: Please return the completed application form with a crossed cheque payable to “Hong Kong Tuberculosis, Chest and Heart Diseases Association” to the Centre. For the application of senior and student member, please send with a copy of your ID card/ Senior Citizen Card or student card to the Centre for checking. Cash is not accepted by mail. 5.            ENTITLEMENT OF MEMBER

 

-              Enjoy special rates for health talks and workshops.

 

-              Enjoy some health checks (include: blood pressure, body mass index and body fat ratio measurement) and health equipment free of charge.

 

-              Access to books, magazines, journals and any health related information.

 

-              Use the computer on searching health information free of charge.

 

-           Receive the association' newsletter, most updated information of workshops and activities.

 

-              Enjoy a discounted price for Chinese Medicine consultation in HKTBA Chinese Medicine Clinic

 

 

6.            USE OF FACILITIES

 

-              Some facilities and services in the Centre are confined to members only. Members are required to present their valid membership cards before using those services and facilities of the Centre.        Members should keep the membership card properly. If lost, please report to the Centre immediately and apply for replacement. HK$60 will be charged for replacement of each membership card.

 

-              Silence and cleanliness must be observed in the Centre.

 

-              No smoking, drinking and eating is allowed in the Centre. (Except for those activities arranged by the Centre).

 

-              Mobile phones, pagers and any other electronic equipment with ringing are prohibited in the Lecture and Meeting Rooms.

 

-              Centre materials are not available for loan and should only be used in the Centre premises. Please enquire the Centre staff for photocopy.

 

-              Computer is used for searching health information only. Registration at the registration counter is required prior to the usage. Users are limited to use for 1 hour. It is an offence for any person to make copies or distribute computer software or audio / visual information from a copyright work without the permission of the copyright owner. Offenders should solely bear the legal responsibilities.

 

 

7.            USE OF EXERCISE EQUIPMENT

 

-              The Center is intended to give users a clean, safe and enjoyable place to exercise for general fitness purposes.

 

-              Users of exercise equipment should know how to operate the exercise equipment and assess their own physical health before use. People with heart disease, high blood pressure and pregnant women should not use the exercise equipment.

 

-              Users should stop exercising immediately and rest in case of any discomfort, such as dizziness, shortness of breath, chest tightness, muscle or joint pain.

 

-              Users should report all equipment malfunctions, personal injuries and specific concerns immediately to the staff.

 

-              Users of exercise equipment in the Centre are at their own risk and the Centre is not responsible for any injury that may occur to individuals participating in any exercise activity.

 

-              The Centre will not provide users any supervision, instruction, observation, first aid, life recovery assistance, or any assistance at all in using any of the exercise equipment of the Centre.

 

 

8.            RESPONSIBILITY OF USERS

 

-              Users must abide by the rules and regulations of the Centre as listed and pay attention to the Centre's staff instructions.

 

-              The Centre reserves the right to request the users to leave the Centre immediately if the users violate the rules and regulations of the Centre, or causing disturbance to the proper operation of the Centre, its staff and other users. The Centre has the right to terminate membership without refund of the paid fees.

 

 

9.            DAMAGE AND THEFT OF CENTRE MATERIALS

 

-              All materials and facilities are the properties of the Centre. It is illegal to remove any materials without permission. Members/ users will be held responsible for any damage to Centre facilities or materials, and will be required to pay all restoration and replacement costs.

 

-              Members/ users should take care of their own personal belongings. The Centre will not be liable for any loss or damage of personal belongings of any user.

 

 

10.          DISCLAIMER

11.          OTHER SPECIAL RULES

 

-              The above regulations notwithstanding, the staff of the Centre has discretion to make special rules on the Centre use as deemed appropriate for the proper management of Centre services and facilities.
